TL;DR: HIFU (High-Intensity Focused Ultrasound) focuses ultrasound at depth to create microscopic coagulation points in the SMAS / deep dermis without touching the skin surface. Ulthera is the FDA-approved classic. Single session, 3–12 months for result maturation, zero social downtime. SMAS targeting = real tissue lifting, not just skin tightening.
Mechanism
Ultrasound (4–10 MHz) is focused at a precise point — a thermal lens. The skin surface passes the energy; at the focal point, 65–70 °C is reached over ~1 mm³ ("Thermal Coagulation Point" — TCP).
Cartridges for different depths:
- 1.5 mm — superficial dermis (skin quality)
- 3.0 mm — mid dermis (skin tightening)
- 4.5 mm — deep dermis / SMAS (real lifting)
The SMAS (Superficial Musculoaponeurotic System) is the target of surgical face-lifts. HIFU reaches the same layer without surgery.
Indications
- Lower face sagging + jowls
- Submental + neck laxity
- Mid-face sagging
- Light brow lift (HIFU brow lift)
- Décolleté aging
- Body HIFU (abdomen, arms, inner thigh)
- Genital tightening (gynae-aesthetic)
Ideal candidate
- 35–60 years, elastin still present
- Mild-to-moderate sagging
- BMI ≤ 30
- Non-smoker
- Realistic expectations
Not ideal: very marked sagging (consider surgery), very thick fat layer, very poor elasticity, multiple existing fillers in face (dissolve first).
Procedure
- Consultation + photo + elasticity assessment
- Topical anesthesia 30–60 min (HIFU is sensitive — anesthesia mandatory)
- Ultrasound gel
- Treatment: handpiece passes over the skin; each shot marked on live target imaging
- Same-day social return; makeup possible
Duration: face 30–60 min; body 60–90 min.
Downtime
- Same-day social return
- Mild erythema 1–3 h
- Some patients: 24–48 h mild swelling
- Mild tenderness 3–7 days
- Rare small bruising
Expected timeline
| Time | Effect |
|---|---|
| Immediate | Mild tightening (collagen contraction) |
| 4–8 weeks | New collagen visible |
| 3–6 months | Maximum effect |
| 9–12 months | Stable |
| 12–18 months | Gradual fade; maintenance |
Side effects
Expected: mild erythema, transient sensitivity, "deep ache" 3–7 days. Less common: transient lower-face swelling, mild bruising, transient muscle fatigue. Rare: superficial burn (wrong parameters), transient nerve sensation (mental, marginal mandibular — typically resolves in 4–12 weeks), surface irregularity, fat atrophy (very rare, sub-optimal use).
Contraindications
- Active skin infection
- Pregnancy
- Pacemaker (relative)
- Metal implant in treatment area
- Autoimmune flare
- Keloid tendency (relative)
HIFU vs alternatives
| Method | Effect | Recovery | Cost |
|---|---|---|---|
| Surgical face-lift | Strongest, 10+ years | 2–4 weeks | ₺200K+ |
| Endolift | Strong, 2–3 years | 3–7 days | ₺50–80K |
| HIFU | Moderate-strong, 12–18 months | None | ₺25–50K |
| Monopolar RF (Thermage) | Moderate, 12 months | None | ₺30–50K |
| Threads | Moderate, 12–18 months | 1–3 days | ₺30–50K |
HIFU's advantage: zero downtime + single session + SMAS-targeting.
HIFU systems
| Brand | Maker | Notes |
|---|---|---|
| Ulthera | Merz | FDA-approved classic, image-guided (DeepSEE) |
| Doublo | Hironic | Korean, more affordable |
| Liftera | Classys | Fast sessions |
| Ultraformer III | Classys | 7 cartridges, broad face + body |
| Sygmalift | Singapore | Low-intensity LIFU variant |
Ulthera DeepSEE visualizes tissue layers during treatment — most precise targeting.
Combinations
- HIFU + Endolift — same-session layered tightening (premium plan)
- HIFU + filler — volume + firming (HIFU first, filler 2–4 weeks later)
- HIFU + Botox — contour + firming
- HIFU + Profhilo — bioremodeling support
- HIFU + RF microneedle — deep + skin quality
- HIFU + threads — synergistic (experienced hands)
FAQ
One session? Usually yes. Some patients have a 2nd at 6 months. Painful? Without anesthesia, hard to tolerate; with topical, comfortable. Summer? Yes — UV-independent. Single-session face-lift? "Lift" is generous — tightening + tissue elevation. Don't expect surgical results. Right candidate + realistic expectation = high satisfaction. Fat reduction? HIFU is firming-focused; LIFU is fat-focused (separate article). Maintenance interval? 12–18 months. Early (6-month) repeats are unproductive. Starting age? Recommended at 35+ with sagging onset, or 30+ preventively.
Our approach
- Skin elasticity + photo assessment
- Ulthera DeepSEE or equivalent image-guided system preferred
- Topical anesthesia + optional IV sedation
- CE/FDA system
- Physician operator
- 3 + 6 month follow-up photos
Conclusion
HIFU is the non-invasive gold standard for 35+ patients with mild-to-moderate sagging. Single session, social-downtime-free, true tissue tightening. Not an alternative to surgery — the strongest result achievable without it.
