RF microneedling, often called the "gold needle" treatment (fractional RF microneedling), is a skin renewal method that delivers radiofrequency energy beneath the skin through gold-plated micro-needles, stimulating collagen and elastin production. It is frequently considered for acne scars, the appearance of enlarged pores, fine wrinkles and a general improvement in skin quality. The benefit gained from the treatment depends not only on the device and the practitioner, but also on the preparation beforehand and the care afterwards.

Who Is RF Microneedling Considered For?

RF microneedling combines the mechanical stimulus of classic microneedling with the heat effect of radiofrequency. The needles pass through the skin surface in controlled micro-channels, while the energy is delivered to the tissue at the targeted depth. This stimulates renewal in the deeper layers without creating widespread damage at the surface. It is most often considered for:

  • Acne scars and depressed (pitted) scars
  • The appearance of enlarged pores and uneven skin texture
  • Fine wrinkles and early-stage skin laxity
  • Support for overall skin quality and firmness

The procedure is not performed, or is postponed, during pregnancy and breastfeeding, in the presence of an active infection in the treatment area, and in people with electronic devices such as pacemakers (in line with the device and manufacturer warnings); in every case, suitability is determined through an examination.

Preparing for the Procedure (1-2 Weeks Before)

Pause Certain Skincare Products

Products containing retinoids (retinol, tretinoin, etc.), strong AHA/BHA acid products and abrasive peels sensitise the skin. You will usually be asked to stop these products 3-7 days before the procedure (according to your physician's advice). If you are taking isotretinoin (a systemic acne medication) or have recently stopped it, be sure to tell your physician; the timing of the procedure will be planned accordingly.

Protect Yourself from the Sun

Performing the procedure on tanned or sunburned skin is not recommended; the risk of pigmentation increases. For 1-2 weeks before the procedure, avoid intense sun exposure and tanning beds, and use SPF regularly.

Tell Your Physician About Your Medications

If you take blood-thinning medication (aspirin, anticoagulants) or supplements such as high-dose omega-3 or vitamin E, be sure to mention this before the procedure. Do not stop any medication on your own — if an adjustment is needed, it will be made by your physician together with the doctor who prescribed the medication. These medications may not prevent the procedure; however, because they increase the likelihood of bruising and pinpoint bleeding, the physician needs to know about them.

Other Conditions to Disclose

  • A history of frequent cold sores (herpes) — preventive medication may be needed
  • An active acne flare-up or skin infection — the procedure may be postponed
  • A tendency to keloid/hypertrophic scarring
  • Pregnancy and breastfeeding — the procedure is generally not performed in these periods
  • Electronic medical devices such as pacemakers (important for radiofrequency devices)
  • Recent procedures such as fillers, Botox or laser

On the Day of the Procedure

  • Your skin should be clean and free of make-up; arriving at the clinic without make-up is practical
  • Your face may be photographed before the procedure (for follow-up purposes)
  • A topical anaesthetic cream is applied for comfort, with a waiting time of about 20-40 minutes
  • The treatment itself usually takes 20-45 minutes, depending on the size of the area
  • A mild sensation of warmth and pricking during the procedure is normal
  • Soothing skincare products are applied afterwards; you can return to daily life the same day

After the Procedure: Day-by-Day Recovery

The First 24 Hours

Moderate redness, a feeling of warmth and mild swelling are expected; most people compare the appearance to a sunburn. Pinpoint redness may be visible at the needle entry points.

  • Avoid touching the skin in the first hours, and keep your hands clean
  • Cleanse the face gently on the first day, only in the way your physician recommends
  • Do not apply make-up (generally not recommended for the first 24 hours)
  • Do not use any products other than the soothing/moisturising products your physician recommends
  • Avoid heavy exercise, saunas, steam baths and hot showers

24-72 Hours

The redness usually subsides noticeably, possibly giving way to mild pinkness and a feeling of dryness. The skin may feel tight during this period. Most people comfortably return to their social life within 24-48 hours; light make-up is generally permitted after 24 hours (according to your physician's advice).

Days 3-7: Micro-Crusting

Fine, grain-of-sand-like micro-crusts and light flaking may appear at the needle entry points. This is a natural part of the healing process.

What Not to Do During This Period

  • • Do not pick at the crusts or peel the skin (risk of scarring)
  • • Do not use peels, scrubs, retinoids or acid-based products (generally for 5-7 days)
  • • Avoid tanning beds and direct sunbathing
  • • Wait the period your physician specifies before swimming pools, the sea and saunas

Weeks 1-4 and Beyond

The skin surface returns to normal, while the real gains begin in the deeper layers. Collagen remodelling progresses over weeks; improvements in skin firmness and texture generally become gradually more apparent over 4-12 weeks. The speed and degree of results vary according to age, skin type and the problem being targeted.

SPF: Non-Negotiable

After the procedure, the skin is temporarily more vulnerable to UV damage and pigmentation. A broad-spectrum sunscreen of at least SPF 30 (preferably SPF 50) should be used every day you go outside, and reapplied during the day. This rule applies not only in summer but also in winter and on overcast days. If sun protection is neglected, the gains from the treatment can be overshadowed by pigmentation problems.

Number and Spacing of Sessions

RF microneedling is generally planned as a course of treatments. Depending on the goal, 3-4 sessions are most often recommended; for structural problems such as acne scars, more sessions may be needed. Sessions are usually spaced 4-6 weeks apart to give the skin time to complete its renewal cycle. To help maintain the results, 1-2 maintenance sessions per year can be considered. The exact plan is determined individually after an examination of your skin.

Combination Options

Where the physician considers it appropriate, RF microneedling can be planned alongside other treatments:

  • Mesotherapy: Can be considered in separate sessions, or within a suitable protocol, to support skin quality
  • PRP: A combination may be considered to support the healing process and skin renewal
  • Botox and fillers: Because they target different concerns, they can be included in the same overall plan, but are generally performed in separate sessions with appropriate intervals

Which combination makes sense depends on your skin type and goals; the equation "more procedures = better results" is not always true.

Daily Habits That Support Healing

  • Moisturising: The soothing and repairing moisturisers your physician recommends support the recovery of the skin barrier in the first week
  • Drinking plenty of water: Skin renewal benefits from good hydration
  • A clean pillowcase: Using clean bedding in the first nights is a simple measure that reduces the risk of infection
  • Avoiding smoking: Smoking adversely affects collagen production and wound healing; reducing it at least during the recovery period is recommended
  • Patience: Collagen renewal takes weeks; it is too early to judge the result in the first days

When Should You Call Your Physician?

  • Redness and swelling lasting longer than 72 hours or progressively worsening
  • Signs of infection: increasing pain, yellow discharge, fever
  • Widespread cold-sore-like lesions
  • Unexpected dark discolouration

These situations are rare; however, early communication allows problems to be managed with simple measures.
