TL;DR: Erbium:YAG (2940 nm) is the gentler, faster-recovery cousin of CO2 laser. Its 10× higher water absorption produces a very superficial, precise ablation. Ideal for periorbital/perioral fine lines, light acne scars, superficial keratoses, and patients who can't tolerate CO2 downtime. Downtime 3–5 days, with markedly lower PIH risk than CO2.
Mechanism
2940 nm sits at water's strongest absorption peak — 10–15× higher than CO2. Result: very superficial ablation (~10–30 µm per pass), very low thermal damage to surrounding tissue, less collagen contraction than CO2. Net effect: precise, controlled scraping rather than deep heating.
CO2 = deep thermal effect. Erbium = superficial precise removal. The right choice depends on target.
Indications
- Periorbital and perioral fine lines
- Superficial acne scars (shallow rolling, boxcar)
- Actinic keratosis, lentigines (superficial)
- Seborrheic keratosis
- Skin texture, pore appearance
- Glogau II–III mild photoaging
- Faster-return patients
Skin types
| Fitzpatrick | Erbium |
|---|---|
| I–III | Ideal |
| IV | Safe with reduced dose |
| V | Limited; fractional mode preferred |
| VI | Not recommended |
Procedure
- Consultation
- Pre-care 2–4 weeks: hydroquinone + retinoid for Type III+
- Topical anesthesia (perioral / periorbital may need local infiltration)
- Treatment: full ablative, fractional, or combined
- Post-care: petroleum ointment, saline compresses
- SPF50+
Downtime
| Day | Status |
|---|---|
| 0–1 | Edema, exudate |
| 2–3 | Crusting |
| 3–5 | Crust falls, pink new skin |
| 5–7 | Pink fades |
| 7–14 | Normal appearance |
Side effects
Expected: edema 24–48 h, exudate 24 h, crust 3–5 days, pink 1–3 weeks. Less common: mild PIH, HSV reactivation, transient sensitivity. Rare: permanent hypopigmentation, bacterial infection, scarring.
Contraindications
- Active HSV (without prophylaxis)
- Isotretinoin within 6 months
- Pregnancy / breastfeeding
- Keloid tendency
- Bacterial skin infection
- Tanned skin
Erbium vs CO2
| Question | Answer |
|---|---|
| Can't tolerate downtime | Erbium |
| Deep acne scars | CO2 (stronger) |
| Periorbital fine lines | Erbium (precise) |
| Comprehensive Glogau III–IV | CO2 |
| Mild Glogau II–III | Erbium |
| Darker skin (IV+) | Erbium safer |
Some clinics combine: deep thermal CO2 + superficial Erbium polishing.
FAQ
5 days enough? Yes for superficial; deeper passes extend to 7–10. Less effective than CO2? Not "less effective" — "more precise." If target is superficial, Erbium is the right tool. Summer? No. Autumn–winter. Multiple sessions? Usually 1–2 for general; 2–4 for acne scars. Anesthesia? Topical for most; local for sensitive areas.
Our approach
- Glogau staging + target zone definition
- HSV prophylaxis
- CE/FDA device (e.g., Sciton Erbium, Fotona, Lumenis UltraPulse Erbium)
- Physician operator
- Day 7 follow-up
Conclusion
Erbium:YAG delivers rapid return + precise superficial resurfacing. For superficial targets and skin types III–IV, often the better choice than CO2.
