TL;DR: Nd:YAG laser (1064 nm) is the only laser wavelength safely usable in Fitzpatrick V–VI skin for hair removal. Deep penetration (5–7 mm) also makes it useful for deep vascular lesions, tattoo removal, onychomycosis, and Genesis non-ablative rejuvenation. Long-pulse and Q-switch versions exist.
How Nd:YAG works
Neodymium-doped Yttrium Aluminum Garnet crystal emits 1064 nm. Key properties:
- Low melanin affinity — safe on dark skin (epidermal melanin absorbs little)
- Moderate hemoglobin affinity — useful for vascular work
- Low water absorption — deep penetration (5–7 mm)
These together allow one device to handle hair removal, vascular, and tattoo in dark-skinned patients.
Long-pulse vs Q-switch
- Long-pulse (ms) — thermal: hair removal, vascular, tightening
- Q-switch (ns) — photomechanical: pigment, tattoo
Indications
Hair removal (long-pulse)
- Fitzpatrick V–VI — only safe choice
- Pseudofolliculitis barbae
- Very coarse black hair (back, chest)
- Tanned skin needing urgent treatment
Vascular (long-pulse)
- Leg spider veins (1–3 mm)
- Reticular veins
- Blue facial vessels (under-eye, chin)
- Lip venous lake
- Deep hemangiomas
Pigment and tattoo (Q-switch)
- Black/blue professional tattoo
- Nevus of Ota
- Solar lentigo in dark skin (only safe option)
- Deep pigmented lesions
Other
- Genesis mode skin tightening — low-dose, high-pass collagen stimulation
- Onychomycosis (nail fungus) — deep penetration kills fungal organisms
- Active cystic acne — low-dose
Skin types
| Fitzpatrick | Nd:YAG |
|---|---|
| I–II | Effective for vascular/Genesis; hair removal less efficient than Alex |
| III–IV | Effective across all indications |
| V | Gold standard for all indications |
| VI | Gold standard for all indications |
Sessions
| Indication | Sessions | Interval |
|---|---|---|
| Dark-skin face hair | 8–12 | 6–8 weeks |
| Dark-skin body hair | 8–10 | 8–10 weeks |
| Leg spider veins | 3–5 | 4 weeks |
| Facial telangiectasia | 2–3 | 4–6 weeks |
| Black tattoo (professional) | 8–15 | 8 weeks |
| Nevus of Ota | 6–12 | 2–3 months |
| Genesis rejuvenation | 6–10 | 2–4 weeks |
| Onychomycosis | 4–6 | 4 weeks |
Procedure
- Consultation, skin type, target defined
- Antiseptic prep (vascular work: alcohol)
- Protective eyewear
- Test patch + 5–10 min observation
- Treatment (long-pulse: seconds per shot; Q-switch: rapid)
- Cold compress
- SPF50+ and required post-care
Side effects
Expected: transient erythema; brief purpura in vascular work (3–7 days); perifollicular edema in hair removal; immediate frosting on pigment. Less common: hyperpigmentation (especially recently sun-exposed), hypopigmentation, transient sensitivity. Rare: burn (energy or cooling failure), scarring, vascular matting (new telangiectasia), Köbner phenomenon in active vitiligo.
Contraindications
- Active sunburn
- Pregnancy / breastfeeding
- Active skin infection
- Isotretinoin within 6 months (for ablative settings)
- Keloid tendency
- Active vitiligo
- Anticoagulant therapy (relative; counseled for vascular work)
Nd:YAG Genesis for rejuvenation
Low-dose, high-pass non-ablative laser toning. Zero downtime; 6–10 sessions, 2–4 weeks apart. Standalone effect is mild — strongest combined with filler, Botox, and Q-switch.
Leg spider veins
- 1–3 mm superficial telangiectasia: ideal
- Deeper varicose veins: sclerotherapy is first line
- 3–5 sessions, 4 weeks apart
- Compression hose + sun avoidance post-treatment
- Doppler ultrasound assessment when indicated
Nd:YAG vs alternatives
| Question | Answer |
|---|---|
| Dark-skin tattoo removal | Nd:YAG Q-switch (or picosecond) |
| Dark-skin hair removal | Nd:YAG long-pulse — only safe option |
| Leg spider veins | Nd:YAG long-pulse |
| Deep hemangioma | Nd:YAG (PDL is first for superficial) |
| Onychomycosis | Nd:YAG combined with oral antifungal |
FAQ
Safe in dark skin? Yes — it's the safe option. Painful vascular work? Rubber-band snap; topical anesthetic helps. Treat tanned skin? More tolerant than Alex/Diode, but still wait 2 weeks. Nail fungus — does it work? Moderate evidence; best combined with oral antifungal. Genesis enough alone? Mild effect. Best combined with other rejuvenation modalities.
Our approach
- First choice for dark skin
- Doppler ultrasound pre-assessment for leg veins
- Test patch + before/after photos
- CE/FDA-approved device
Conclusion
Nd:YAG is the most versatile member of the laser family: dark skin, deep vascular, deep pigment all in one. Correct parameters + cooling + experienced operator deliver an excellent safety profile.
