Are biostimulatory fillers and hyaluronic acid fillers the same?

Hyaluronic acid (HA), calcium hydroxylapatite (CaHA), poly-L-lactic acid (PLLA), PMMA and other injectables differ in carrier, particles, tissue response, onset, persistence and complication management. A claim that a product stimulates collagen cannot replace product identity or an assessment of suitability.

  • Hyaluronidase used for HA is not a universal antidote for particulate or biostimulatory products.
  • Immediate volume and a tissue response developing over time require different outcome and follow-up models.

Why material behaviour matters

HA gels provide volume and shape through water interaction, cross-linking and viscoelastic properties, and their formulations are not interchangeable. Products such as CaHA and PLLA may combine a carrier effect with a cellular and foreign-body response that develops over time. Permanent particles such as PMMA have a different timeline and different limits on removal.

Dissolution and reversibility are not the same

HA can be broken down with hyaluronidase in selected circumstances, but the result depends on dose, location, timing and complication type. CaHA, PLLA, PMMA and silicone do not dissolve with the same enzyme. Gradual biodegradation also does not mean that a product can be easily reversed in an emergency. A patient card and lot record are therefore important for later care.

How to discuss product choice

First define the anatomical target, skin and tissue thickness, previous materials, desired timeline and risk tolerance. Then review the exact product, Turkish registration and indication, planned depth and volume, follow-up and possible complication response. Popularity or longer duration in one class is not proof of superiority or a guarantee of personal benefit.
