Some cohorts report fewer vascular occlusions with cannulas, but a cannula does not eliminate vascular events and is not suitable for every area, product or target. Choice requires anatomy, injection plane, product, entry point and practitioner experience to be considered together.

In a practitioner-reported cohort covering 1.7 million syringes, reported vascular occlusion was lower with cannulas than needles. The study was not randomized and may be influenced by practitioner selection, experience, area and product. Its rates are not a personal-risk calculator and do not exclude rare outcomes such as visual loss.
Needles and cannulas differ in entry control, tissue passage, product placement and tactile feedback. The nose, glabella, lips, temple and periocular area do not share one vascular anatomy. Intended plane, product rheology, previous filler and scarring also change the decision. A statement that a clinic 'uses cannulas' is not a risk assessment.
Discuss area-specific anatomy training, the chosen instrument and rationale, controlled product placement, recognition of vascular signs, access to hyaluronidase for HA products only, and urgent transfer for visual symptoms. No technique, aspiration method or imaging tool provides complete protection by itself.
