Maintenance may cover cables, handpiece and transducer recognition, software, cooling or power components according to the manufacturer. What calibration means and how often it is required differ by system. A serviced device does not prove that the proposed area is suitable or that depth and energy are correctly planned. Examination, anatomical knowledge and device-specific training remain separate safety layers.
The document should name the manufacturer and exact model, serial number, service date, service organization, replaced parts, test method, result and next due date. An unsigned note saying only 'serviced' offers little traceability. If software changed, record the version, manufacturer notice and effect on user training.
