How are hypopigmentation, hyperpigmentation and scarring distinguished after laser treatment?

Darkening, lightening and textural change after laser treatment are not the same complication. Timing, borders, surface texture, wound healing, infection, sun exposure and baseline skin tone need joint assessment; an early photograph cannot establish permanence.

  • A new blister, open wound, discharge or increasing pain needs early assessment.

Three different outcomes

Post-inflammatory hyperpigmentation is darkening related to melanin; hypopigmentation is lightening from reduced pigment or melanocyte injury; a scar changes collagen and architecture. They may coexist. Wood lamp, dermoscopy, palpation or biopsy depends on the clinical question. Camera white balance and tanning can create a false colour difference.

Risk context

Wavelength, pulse duration, fluence, overlap, cooling, target condition, skin tone, recent tan, healing behavior and infection all influence outcome. Phototype is useful but incomplete. Systematic reviews contain heterogeneous devices and definitions, so an average rate cannot establish personal risk or the safety of one setting.

Documentation and follow-up

Match camera, light, distance and settings at baseline and follow-up, and separately record elevation, depression, firmness, itch and pain. Preserve products, sun exposure and interventions.
