A negative aspiration does not prove that a needle or cannula tip is outside a vessel. Results are affected by product flow properties, needle diameter, waiting time, contact with the vessel wall and movement of the tip during injection.

Laboratory studies show that time to visible reflux changes with a filler's elastic properties, cohesivity and needle lumen. Waiting too briefly, slow blood movement through a dense product, or the tip resting against a vessel wall can create false reassurance. The tip can also move after testing when injection begins.
A negative test does not mean 'the tip is not intravascular,' 'this area is safe' or 'rapid injection is acceptable.' Feasibility can also differ with cannulas and connector systems. Evidence does not show that aspiration alone prevents visual loss or tissue necrosis.
The exact product and area should be verified, with relevant vascular anatomy, prior procedures and scarring assessed. Controlled pressure, careful tissue observation, recognition of pain and colour-change signals and a situation-specific emergency protocol matter.
