How should treatment targets, eye safety and evidence limits be read together for periorbital HIFU?

Periorbital HIFU studies examine different outcomes such as brow or eyelid contour, wrinkles and elasticity; they do not support exposing the globe to ultrasound. Exact labeling, distance from the eye, bony boundary, transducer depth and individual anatomy take priority.

Name the outcome in the study

A recent review contains small, heterogeneous studies measuring wrinkle grades, elasticity, surface topography or brow and eyelid contour. These outcomes do not have the same clinical meaning. Summary results cannot transfer to another protocol without device, transducer, anatomical area and protection details. The word periorbital is not authorization to treat over the eye.

Anatomical and labeling boundaries

Assess the orbital rim, globe, mobile eyelid, nerves, vessels and possible bone reflection. FDA guidance warns against exposing eyes to ultrasound radiation. A standard template cannot show personal orbital anatomy or prior surgery. When a device has no periorbital indication, a paper or training video does not replace current local instructions.

Do not defer an eye symptom

New blurred or double vision, flashes, a visual-field change, marked eye pain, impaired eyelid closure or weakness should not wait for routine follow-up. Record time, side, area, transducer and parameters, and arrange appropriate ophthalmic or neurological assessment. Absence of an immediate symptom does not by itself prove long-term ocular safety.
