Is HIFU the same across skin tones, and how should pigment risk be assessed?

Focused ultrasound is not a light-based laser and melanin is not its primary energy target, but that does not mean zero pigment risk in every skin tone. Device, contact, energy, surface heating, pigment history, co-procedures and representation of skin types in the evidence all need review.

Direct answer

Not targeting melanin does not make burns or post-inflammatory pigment change impossible.
Including several phototypes in a study does not guarantee adequately powered safety analysis for every subgroup.

A different energy mechanism with residual risk

HIFU energy does not rely on light chromophores; it produces mechanical and thermal effects at a focus. Laser rules about melanin absorption therefore do not transfer directly. Yet poor contact, an inappropriate transducer, superficial energy deposition, crusting or inflammation can be followed by colour change. Neither the device label nor skin tone alone supplies a safety conclusion.

Systematic reviews combine studies from several countries, but phototype distribution, subgroup events and pigment outcomes are not detailed in every paper. Even a seemingly large total sample may be underpowered for a rare event in darker skin. Ask whether the exact device and protocol were studied in people with a similar skin tone and whether outcomes were actively collected.

Personal risk and follow-up

Document melasma, post-inflammatory hyperpigmentation, active dermatitis, recent tanning, sensitising medicines and recent laser or peel procedures. Standardized photographs and the onset date of symptoms matter in follow-up. Persistent redness, blistering, crusting or colour change warrants early clinical assessment rather than starting a new active product from online advice.
