Why are dermal fillers not routinely planned during pregnancy or breastfeeding?

Dermal filler safety during pregnancy and breastfeeding has not been established in controlled studies, and current FDA information describes it as unknown. Why is the evidence limited?

Randomized studies of cosmetic injections in pregnant people are difficult for ethical and practical reasons. The literature therefore relies heavily on indirect evidence, reports and expert assessment. Local placement does not define a pregnancy safety threshold for cross-linker, lidocaine, an animal- or bacteria-derived component, infection or a vascular complication.

Breastfeeding and the complication plan

Product-specific information on transfer into milk, infant effects and different filler materials may be absent. A complication can also create a second decision involving an antibiotic, antiviral, antihistamine, steroid, imaging or emergency care. A low probability does not make an elective intervention necessary during that period.

What if exposure already occurred?

Receiving filler before recognizing a pregnancy does not by itself establish harm and is not a reason to panic. Record the product, lot, area, date, anaesthetic and symptoms, then discuss them with the obstetric clinician and injector. For any new elective treatment, review the current product label, personal health context and the option to postpone explicitly.
